Cancellation
Appointments canceled 24 hours or more before the start of your scheduled visit will not be charged. If you fail to cancel within 24 hours, a $25 fee will be made to the credit card on file.
No Shows
A $50 will be charged to the credit card on file for patients that do not show up for their scheduled appointment.
Late Shows
We value your time and that of other patients. Please make every effort to be on time. We will try to accommodate you if we have any openings that day, but as a consideration to other patients, we will not be able to see you if you are more than 15 minutes late for your visit.

We accept all major credit cards, debit cards, and checks.
Termination of Physician-Patient Relationship
We reserve the right to terminate the physician-patient relationship for disrespect, mistrust, or behavior that puts the patient, the treating physician, the staff, or other patients in practice in jeopardy. A formal letter of termination will be provided.
